14  Observed the following improper food contact surfaces: 1) wood pizza peels with chipped/frayed wood. Provide for all food contact surfaces to be smooth, easily cleanable, non-absorbent, corrosive resistant, and light colored. Wood as a food contact surfaces is prohibited. (750.600) Next Inspection
15  Observed the following improper non-food contact surfaces: 1) rust on wire racks in corner of kitchen, 2) rust on wire racks above 3-bay sink, 3) rust on bottom shelf of kitchen prep table, 4) chipped wood on lower shelf of table in kitchen, 5) torn gaskets on 2-door cooler in bar. Provide for all food contact surfaces to be smooth, easily cleanable, non-absorbent, corrosive resistant, and light colored. Repair, repaint or replace. (750.690) Next Inspection
23  Observed the following contact surfaces to be unclean: 1) fan by 3-bay sink in kitchen, 2) inside of drawer to prep table in kitchen, 3) clean gaskets of pizza prep cooler, 4) shelving with chemicals and single service items in back room, 5) inside back panel and ledges of ice machine in back, 6) all shelving in walk in cooler. 32  Observed no paper towels in restrooms. Provide for all hand sinks to have supply of soap and paper towels to assure that proper hand washing occurs. *NOTE: this code changed in 2009 where it states that all hand sinks including those in restrooms must have paper towels regardless if restroom has automatic hand dryers. This is to ensure that proper hand washing is taking place and that customers have the ability to open the door with a paper towel. This is now classified as a #12 violation which results in a major violation if the facility does not have paper towels in the restrooms. (750.560) Immediate/Onsite

Provide for all flooring areas to be smooth, easily cleanable, non-absorbent, durable surfaces, and be maintained in good repair. (750.1200) Observed the following floor areas to be unclean: 1) in bar, under and around equipment. Provide for all flooring areas to be in a clean condition. (750.1220) Next Inspection
37  Observed the following wall/ceilings area(s) to be in poor repair: 1) chipped paint on kitchen walls, dry storage walls and walls in room with water heater, 2) improper surface on walls and ceiling in walk in cooler, 3) chipped/fraying surface on walls and ceiling in back hallway. Provide for all walls and ceilings to be smooth, easily cleanable, non-absorbent, durable, and light colored and be maintained in good repair.
